Patient Handouts for Healthcare Providers

At the Middlesex-London Health Unit, we provide handouts on a wide variety of public health topics to support the information shared during visits with patients.

The MLHU Resource Binder is now online!

We’re excited to announce that the MLHU Resource Binder has been updated and moved to a new, convenient online platform—the Public Health Ordering System (PHOS). This system allows you to easily view, download, print, and order resources for your office and patients.

What is PHOS?

PHOS is a one-stop online platform for Healthcare Providers in Middlesex County and the City of London to access and order publicly funded vaccines and public health resources. Those who order vaccines from MLHU will already know this platform. For those new to PHOS, it is an easy-to-use system to learn. Whether you’re looking for patient handouts, Health Unit forms or point-of-care tools, PHOS makes it easy to find and order what you need, all from the comfort of your clinic.

Accessing PHOS:

To access PHOS, you must first create a personal account that is tied to your clinic. This ensures you have secure access to all the resources available.

 

User Guide

How to Get Started:

Your account will allow you to view, download, print, and order resources for your office and patients.

1. Already Ordering Vaccines?
If your office orders publicly funded vaccines from MLHU, someone in your office already has a PHOS account and can add you as a user (See, how to manage users).

2. The Only PHOS User in Your Clinic?
If you’re the sole PHOS user in your clinic:

  • Expect requests from your office members to add them as users.
  • Follow these instructions for adding/managing users.
  • New users will receive an invitation they need to accept for security reasons (see, how to sign up for help). This process may take up to a few business days.

3. New to PHOS and Not Providing Vaccines?
If your healthcare provider office doesn’t provide vaccines and isn’t set up with PHOS yet:

  • Have one member of your office staff sign your clinic up here.
  • This individual will be the contact responsible for adding initial users.
