The City of London provides a subsidy for child care under certain circumstances. They have asked that we share the following information with health care providers.

Child Care Fee Subsidy is available through the City of London for families who meet the financial eligibility criteria and are employed, attending school (ESL, secondary or post-secondary) or have a Therapeutic Referral for their child or themselves as parents/guardians. Placement for children based on a Therapeutic Referral requires the submission of a Referral Form and Consent Form. Alternatively Physicians/Family Health Teams can provide a letter with the details as outline in the Referral Criteria for Physicians/Family Health Teams. (Attached).
